Font of choice i used Bank Gothic Md Bt.
Open template delete DBA layer.
Click on transparent part of template image
canvas size 600x700 will resize later.
New layer fill with white layers arrange send to bottom.
My method for filling layers...
Highlight layer you want to fill selections select all
selections float selections defloat new layer fill with colour of choice.
Add selection of effects to your own liking delete original layer.
Click on middle of rectangle layer to highlight it
magic wand click in center.
paste tube of choice selections invert delete on keyboard.
Paste tube again drag to top drop shaddow line up with 1st tube
use selection tool to cut bottom overlapping of tube away.
Add wordart to long outer rectangles and bottom center rectangle
eyecandy gradient glow or white drop shaddow then normal drop shaddow.
Drop shaddow all layers.
Add name place inside left of tube drop shaddow rotate.
Add url copywrite and watermark.
New layer move to above white back ground fill with colour of choice
mask new from image select and apply mask merge mask visable.
Resize crop delete white background merge layers visable.
Save as PNG.
