Font of choice i used Aharoni.

File new 600x650 fill with white new layer fill

with paper of choice mask new from image select and apply mask

merge mask layer visable.

Rersize frame23 paste as new layer on top of mask magic wand

click in centre selections modify exspand by2 paste tube of choice

selections invert delete on keyboard drag below frame

drop shaddow tube and frame select none.

Resize rose8 paste as new layer place

to top right of frame drop shaddow.

Resize heart12 paste as new layer place to

bottom right of frame drop shaddow.

Resize heart17 paste as new layer place

to bottom left of frame drop shaddow erase a small bit

at each end of string.

Use preset shapes tool draw ellipse around tag

see my labels help tuts how to.

Add wordart drop shaddow to top left of tag.

Add name place to bottom of tag drop shaddow.

Add url copywrite and watermark.

Delete white background merge layer visable.Resize crop.

Save as PNG.
