Font of choice i used Alex Brush.

Penta.com dot and cross plugin.
Open template delete left and right squares.

Highlight large circle duplicate select all float new layer fill with black

image resize 103 drag below original square select none.

Repeat for the other two circles but resize by 101.

Method for filling layers...

Highlight layer your wanting to fill select all float

new layer fill with either a gradient or colour of choice.

Delete original layer select none.

Large circle select all float open close tube of choice

paste as new layer, i made my tube big enough to fill circle.

selections invert delete on keyboard.

Change blend mode to soft light add penta.com dot and cross twice.select none.

Highlight black frame on large circle add noise mix set 100.

Highlight left rectangle select all float paste tube again

to your liking invert delete on keyboard select none

change blend mode to soft light.

Repeat same for right circle add soft light select none.

Copy and paste main tube place to center on top of all circles.

Drop shaddow twice.

I added psp weave to the Scandalous wordart.

Highlight giv em something duplicate select all modify exspand by2

new layer fill with black drag below original layer select none.

Add url copyright and watermark.

Add name i used text size 72 place to right center 

on top of template drop shaddow.
